PERSONAL SERVICE AGREEMENT
BETWEEN
CANTON PUBLIC SCHOOLS
AND
NELSON D. BATCHELDER, M.D.
1030 WASHINGTON STREET, CANTON, MASSACHUSETTS 02021

Dr. Batchelder will serve as the School Physician from July 1, 2015 to June 30, 2018.
During this period of time his duties and responsibilities shall include the fallowing:

G.      Student physical examination for High School student athletes, kindergarten, grade 5
and grade 9 students whose parents cannot afford to use their family physician.

H.      Examination of all students and staff referred for health reasons.

l.       Serve as a consultant to teachers, administrators, and school nurses for health
problems as they relate to school and other activities including emergencies, schoo!
safety, and athletic safety.

J.      Provide, in conjunction with the Department of Public Health, a program of immunization
against communicable diseases, control of parasite infestations, detection and control of
tuberculosis for all student and staff.

K.      Respond to all correspondence and inquiries regarding school related health matters.

L.      Administer mandatory physical examinations to all school bus drivers employed by the
Canton Public Schools at a rate of $350.00 for each of the three years of the Agreement

The School Physician shall participate in the town sponsored health plan for the period of July
1, 2015 through June 30, 2018.

The School Physician shall receive an annual stipend of:

2015 - 16          $9325.22
2016 - 17          $9511.73
2017 — 18          $9701.96

The School Physician shall report directly to the Superintendent of Schools. The School
Physician will submit a contact log on a monthly basis to the Superintendent.

This Personal Service Agreement is subject to School Committee appropriations and approval
of the School Department Budget at the Annual Town Meeting.
