Plan Title: Alta at Indian Woods Driveway Modification
Plans dated December 8, 2010
Plans Stamped By: Douglas Hartnett, P.E.

Project was sold January 10, 2010 to Wood Partners. Existing conditions plan to include utilities
based on a progress as built survey titled Utility As- Built Along Access Way located in Canton,
Massachusetts” dated October 27, 2010 by Sullivan Engineering Group Inc. Proposed plans
reflect minor modifications from the approved plans.
Following modifications are:

• Plans show new owner, applicant, and surveyor.
• Secondary entrance identity sign is proposed to be installed in the northwest corner of the
entrance driveway. Sign foundation will be approximately 63 feet north of the existing
wetland.
• Existing hay bale/silt fence erosion control barrier will be modified to extend around the
limit of work for secondary sign.
• As-built location of the new riser pole and the electrical duct bank were added to the
plans.
• As-built location of the new 8” water main was added to the plans.
